+ I’m new. What paperwork do I need?
Clients may fill out a New Client Form to save time at the counter.
+ My pet is having surgery. What do I need to know?
If your pet is coming for surgery please review, print, and initial the Surgical Information Packet.
+ Appointment and Prescription Policies
To maximize appointment availability JVC has implemented the following appiontment and prescription polices:
- Cancellation Fees: No call/no show and short notice cancellations (less than 24 hours prior to appointment) $40.
- "Work-In" Appointments: Same-day appointments requiring a work-in $25.
- Emergency Fees: Emergencies disrupting normal scheduling $80.
- Medication Pick Up: All medication pick-ups require 24 hour notice to be filled. These must be picked up by Saturday or a $10 restocking fee will be charged, unless medication is paid in advance.
- Nail trim only appointments are no longer available.
- All patients must have an updated rabies vaccine on file.
