+ I’m new. What paperwork do I need?

Clients may fill out a New Client Form to save time at the counter.


+ My pet is having surgery. What do I need to know?

If your pet is coming for surgery please review, print, and initial the Surgical Information Packet.


+ Appointment and Prescription Policies

To maximize appointment availability JVC has implemented the following appiontment and prescription polices:

  • Cancellation Fees: No call/no show and short notice cancellations (less than 24 hours prior to appointment) $40.
  • "Work-In" Appointments: Same-day appointments requiring a work-in $25.
  • Emergency Fees: Emergencies disrupting normal scheduling $80.
  • Medication Pick Up: All medication pick-ups require 24 hour notice to be filled. These must be picked up by Saturday or a $10 restocking fee will be charged, unless medication is paid in advance.
  • Nail trim only appointments are no longer available.
  • All patients must have an updated rabies vaccine on file.

Yes! Check out our online store! You may fill prescriptions, order food, toys, and more! You can even set up auto-delivery so you won’t have to remember to order!
